Online Application
You can fill out and submit your forms online. You will need an internet connection in order to do this. Once your forms have been received, you will get back an email confirmation. The forms are quite lengthy; however, you may save your form at any time and resume filling it out at a later time. Begin the online application *Please note that we will review all submitted information and appropriate candidates who look like they will benefit from UMAP then will be invited to attend. If you decide to accept the invitation to UMAP, enrollment is on a first-come, first-served basis.
