How to Apply

In order to apply to the UMAP, download and print out the following application form. Remember to fill in each section as completely as possible.

Mail the completed application with any supplemental information to the following address:

In addition to your application, have your physician complete the Medical Information Form found in back of your application materials. If you have copies of hospitalization and rehabilitation service records, the application process will be expedited by forwarding them along with the application.
